Even worse today. Saw the dermatologist (not the same one) who seemed to rush the appointment, didnt want to hear what I had to say and is KEEPING ME ON THE SAME DOSE WITHOUT EXPLANATION! Is this normal? I thought everyone got doubled up? Am I to stay on 50mg for the whole 4 months even though I have seen NO CHANGE?! I feel so helpless and hopeless now. Im sorry for being so depressing :-( Nikki x

Hey - think I just left you another blog.

Don’t panic, 50mg is a fairly high dose so you’ll probably stay on it for a while. Some people go to 60mg and I have heard of very very few people having 80mg, but I think you are already quite high, so don’t panic.

These things take time, the body is a complex bit of kit and it can’t make a U-Turn over night. Stress is a frequent cause of acne so try to relax yourself as best you can and do things to distract yourself.

I think they can only put you on an absolute max of 1 mg per kilo of your body weight. I started on 40 mg which is fairly high as lots start on 20mg (one person i read was on 5!). My derm told me if they increase it too quickly then you cant stay on the tablets very long. You started just before me so will maybe (like me) be on 40 mg for a min 4 months then 60mg for a max of two months at the end.

Talk to you Derm about the cumulative dose. You should be looking to achieve your cumulative dose. It has been suggested (by the makers Roche) that a cumulative dose between 120 – 150mg / Kg will result in a longer period of remission as compared to stopping when acne is first brought under control by Roaccutane.
You work out your cumulative dosage by taking your dosage for each month & multiplying it by how many days in the month. Once you have this number you divide it by your weight. My cumulative dose is (I weigh 70kg)…

Month 1 – 20/40mg * 28 = 840mg
Month 2 – 60mg * 30 = 1,800mg
Month 3 – 60mg * 30 = 1,800mg
Month 4 – 60mg * 30 = 1,800mg
Month 5 – 60mg * 30 = 1,800mg
Month 6 – 60mg * 30 = 1,800mg

Total 9,840mg / 70kg = 140 mg per kg (so falls into the 120/150 mg/kg bracket)

Hi Nikki,

I can sympathise with you lots hun. Im on month 3 and have seen NO improvement at all. To be honest, I think i look much worse )-: aswell as spots, i also now have scars and red marks (do these go???) all over my face which i didnt used to get PRE-RO-ACC. My dermatologist is the same - rushes the appointment and said she cant up my dose and im only on 30mg )-: Despite the fact i have seen no dryness what soever.

Im so Depressed right now, I honestly cant remember a day that I havent cried becuase of how i look. Have u seen any changes at all? I wish i was on 50mg! Arghhhhhhhh

So Im right there with you chum (-;

Vicky/ Nikki, thats awful

Can you not try and get a refferal to another derm or see your GP? My doctor is really nice in that she listens to what I have to say. She did tell me that you may only start to see improvements after the course of treatment. I have also been told this by one of my friends who used the drug. I am so with you Vicky regarding the whole “hating the way you looks”. Rememeber though that looks are only skin deep. Heck that probably doesnt help, presumably of you are feelng depressed- you have to ask the question, is the drug worth it for you. Have you tried any other drugs?

Hi Mark,

Yeh ive tried everything i think there is out there. Finally got a referral from the Doc’s to go see a Derm and i was delighted thinking that things were going to get better. My Auntie was on Ro-Acc last year and her skin is perfect and was so after 2 months or something.

Im just getting so fruhstrated with things now - and its ruining my life!!!!! Surely I would see some improement by now? I just wish my skin was dry or something you know what i mean?! I wake up every single morning without fail and have 2/3 new spots to add to the existing ones! Its ridiculous! My Derm is nice but she just refuses to increase my dose!

Sorry for the rant Mark - Im just feeling really down! )-: What about you? What stage you on and are you seing any improvement?

Hi Nikki,
Try not to worry. I have been on 20mg ONLY for 4 months now!! You started on 50mg which is quite high in itself!

I’m sure you might be sick of hearing this, but everybody reacts differently to roacc. Some people notice lots of changes and improvements early on and some don’t for a long time. I had a long period from week 5 to week 11 where my skin just got worse. I am starting to see improvement but even at this late stage I still get new big spots and scars each week and I have not had any dry skin!

Give roacc time and try not to think about it too much or it really will get you down!. At least you are doing something to help get rid of your acne, and what is a few months?! If we were not taking it, we would still have these spots anyway.

I no excatly how you feel. I have been having really sore acne or what 3-4 years now and iv been on everything possible, all the lotions and tablets.

What are you using on your face or wash. I have be reccomended Dove soap- This has really helped in drying the skin and then I use a Simple light cream.

I have noticed improvements from the beginning- its now week 7. Skin and lips are terribly dry especially lips- all cracked and bleeding- but use a cream and it will be fine.

I am also getting rashes on my arms which is crap??!?
and bleeding noses and stomach aches.
